As occasionally happens with any dental procedure, a tooth may not heal as expected after the first treatment for different reasons, such as:   – Narrow, blocked, calcified or curved canals were not treated during the initial procedure. – Complicated canal anatomy went undetected in the first root canal. – The placement of the crown or proper restoration was delayed following the endodontic treatment. – New decay can expose the root canal filling material to bacteria, causing a new infection in the tooth. – A loose, cracked or broken crown or filling can expose the tooth to new infection. – A tooth sustains a fracture. – Saliva entering the tooth from the leaky restoration.   Research has showed that the success of the root canal treatment is dependent on the proper final restoration with proper filling and seal. Utilizing proper lighting and magnification will increase the success rate of the first root canal treatment. Last year I had a patient and the canals were not cleaned properly with the previous dentist. So we have to do retreatment for this patient. I reopen the tooth and remove the filling materials that were placed in the root canals during the first procedure. I carefully checked the tooth, and looked for additional canals or new infection.  I found one missed canal on the distal of the tooth, other 3 canals were not cleaned properly. The tooth had infection, fistula and it was mobile. I removed the old root canal filling material and then cleaned and shaped the canals, and places new filling materials. The opening was sealed with amalgam filling and patient will receive a crown. In some difficult cases patient should referred to endodontic or root canal specialist.
